Indications
1. People with respiratory insufficiency and no contraindications can try a non-invasive ventilator.
1) Acute respiratory failure and acute exacerbation of chronic respiratory failure.
2) Acute cardiogenic pulmonary edema.
3) Persistent asthma.
4) Transition before weaning of invasive ventilation.
5) Neuromuscular diseases.
6) Sleep apnea and obesity hypopnea syndrome (Pickwickian Syndrome).
2. Non-invasive ventilation can be used as the first-line treatment for patients with acute exacerbation of COPD and acute cardiogenic pulmonary edema.
3. Patients with respiratory failure with immunosuppression can try non-invasive ventilation first.
Contraindications
1. Absolute contraindications:
1) The heartbeat stops breathing.
2) Weak spontaneous breathing and coma.
3) Patients who are prone to aspiration.
4) Combined with failure of other organs (hemodynamic instability, gastrointestinal hemorrhage/perforation, severe brain disease, etc.).
5) Facial trauma/postoperative/deformity.
6) Uncooperative patients.
2. Relative contraindications:
1) Excessive airway secretions, sputum expectoration disorder.
2) With severe infection.
3) Extremely nervous patients.
4) Severe hypoxemia (PaO₂<45mmHg, severe acidosis pH≤7.20).
5) After recent upper abdominal surgery (especially those who need strict gastrointestinal decompression).
6) Severely obese patients.
7) Patients with mechanical obstruction of the upper airway. In units with better monitoring conditions and rich experience, under the premise of close observation, it can be used as an exploratory application to patients with relatively contraindications.
